Birdseye Posted June 18, 2015 Posted June 18, 2015 When I was in the job, the main idea was to prevent collisions. Intersecting runways, poor speech articulation, speedy/clipped transmissions and callsign confusion are a combined recipe for disaster. 1
astroman Posted June 19, 2015 Posted June 19, 2015 This kind of incident is scary because it is not a case of somebody failing to adhere to protocols, but simple human error. The kind of error that causes huge crashes with loss of life. Good thing the controller was on the ball in the end!
Birdseye Posted June 20, 2015 Posted June 20, 2015 A thorough investigation would show a number of causal factors, mostly systemic and related to poor ATS management. I'd suggest that human error to the event, but was not a root cause. 1

facthunter Posted June 20, 2015 Posted June 20, 2015 Use of "twenty eight" Instead of Three, two, eight and Eight two eight. Better to use company names ie Delta eight two eight. Pick up on the read back should happen also. An awful lot depends on it. When you are using simultaneous intersecting runways, it's deadly. Nev
