I wonder which member of the ground crew failed to pull out the safety lock on the seat before releasing the aircraft for use.

I think his career & credibility are in tact, it’s called flexibility, I’m sure there’s no QRH procedure that deals with a fire plus crew member seat u/s.

It's amazing what can happen when you roll the dice.

 

I do reckon they need to check more than the seat safety pins.
